site, as measured in “page views,” is the Pennsylvania School
Affidavit (Continuous Residency and Support Oath) available
for download and/or printing. This notarized affidavit
states that an individual, other than the parent(s) of a
minor, is providing all financial support to the minor, and
will be responsible for the minor's school attendance and
behavior. This form enables a minor to attend school in a
district other than where his/her parents reside. |
